Duties of Caregiver Support Program Coordinator Jobs:

Manages information and analytical systems to assess and improve the quality of service offered to veterans, families, and caregivers. Develops and executes information systems to monitor service activities, such as referrals, visits, caregiver demographics, equipment utilization, and satisfaction of both caregivers and veterans. Establishes the appropriate utilization of services by developing, implementing, and evaluating clinical guidelines and protocols.

Engages in direct communication with veterans and caregivers to evaluate and identify caregiver stress, as well as to develop and implement directed clinical treatment interventions. Provides guidance and collaborates with interdisciplinary teams across the medical center regarding caregiver concerns. Develops programs, implements training, and creates educational materials that address specific caregiver concerns. Offers education and in-service training to VAMC employees, veterans, families, and caregivers.

Referrals are provided to VA medical and mental health treatment, as well as other VA services, including VA benefits, community-based social services, and other non-VA entitlement programs. The coordinator will provide referrals or consults, or in some cases, direct admission of a veteran to other VA programs, such as the Community Residential Care Program, Respite Program, Community Health Care Coordination Program, Hospice, Home-Based Primary Care, Telehealth, and Community Nursing Home Program, if appropriate.

Will collaborate with the VA Patient Aligned Care Team (PACT), mental health, and geriatric primary care clinicians to offer education on caregiver issues to Veterans and staff. A repository of caregiver support information materials will be created for distribution in a variety of inpatient/outpatient and specialty clinic settings.

Acts as a liaison between community agencies and the VA. State and community caregiver-related agencies are invited to participate in the local VA Caregiver Support Advisory Board. Establishes and cultivates relationships with community partners to facilitate system navigation, maintain collaboration, and coordinate services. Provides outreach presentations to community social service agencies, military Guard and Reserve units, veteran service organizations, and other groups regarding the benefits, programs, and services of VA caregivers. In collaboration with the local VA Public Affairs Officer, the VA Caregiver Support Program is promoted through interviews with local print, voice, and television media. Maintains a fundamental understanding of the local VA medical center programs, community services, and VA Central Office caregiver support initiatives.

I will establish a caregiver support protocol (resources, education, and training) in collaboration with other clinical staff and program administrators that is sensitive to the issues of the caregivers who are providing care to our youngest returning veterans.

Coordinates the events and programs for the annual National Family Caregivers Month in November, which includes the announcement and advertising of month-long activities, the mobilization of participants, educational sessions, caregiver recognition events, and caregiver support programs. Assess the program’s progress and results, and employ the data to perpetually enhance and modify the program and processes.
